Medical Weight Loss

Initial consultation includes: labs, history, and basic physical, to ensure medication management is safe and appropriate for weight loss. Guidance is provided for diet and activity and if appropriate a prescription will be sent to Lilly Direct for first month supply of Zepbound if a GIP/GLP-1 is appropriate. Please note: consultation does not include the cost of medication or supplies for administration. Initial consultations are in person only at this time. 

Appointments are intended to ensure medication is effective, discuss side effects, & make dose adjustments as necessary. A refill for 1-month (28 day supply) will be provided if appropriate. Appointment does not include the cost of medication or supplies for administration.

Note: no dose adjustments will be made again till you next 3 month follow up. Appointment does not include the cost of medication and supplies for administration.

As needed visit outside of normal Weight loss/Men's health membership appointments. These appointments are intended for additional coaching or to managed side effect and health concerns related to medical weight loss and/or men's testosterone treatment only. Any general health concerns should be scheduled with your primary care provider. Appointments required for any questions or concerns that can not be addressed at routine visits and are billed at $50.00 per the weight loss clinic policies.

Oral medications can be used in place or in addition to GIP/GLP medications. Options vary based on patient medical history and needs. This option can be discussed at your consultation or at any follow up visit if you are interested in an oral medication to determine if it is appropriate. Please note: we do not prescribe oral semaglutide. 

You are welcome to use your HSA/FSA. If you should require a superbill  there is  surcharge for creating this document. Please note that while we are happy to provide a superbill for your records, submission of a superbill does not guarantee reimbursement or approval of the charge through your HSA/FSA account. Coverage and eligibility for reimbursement are determined by your individual plan provider, so we recommend confirming directly with them for specific requirements.
