In person consultation for men's testosterone replacement in Uniontown, Ohio. Includes cost of initial labs. Required for all new members.

Consultation are done in two parts. First, at time of lab draw (you can ask all your questions at this time) Second, is at time of treatment planning.

Review: symptoms, risk/benefits of treatment, & discuss treatment options. Review of labs completed in office at first visit.

Routine follow up labs

     Labs are necessary at 3, 6, and 12 months after initiation of men's testosterone replacement treatment or anytime a dose change may be necessary. This ensures medication is effective and safe.

Lab fees can be avoided when selecting a Men's Elite package *limit 4 lab draws in a 12 month period.

Includes unlimited follow up visits for men's testosterone replacement . 

Routine labs up to 4x a year as needed.

As needed visit outside of normal Men's health membership appointments. These appointments are intended for additional coaching or to manage side effect and health concerns related to men's testosterone treatment only. 

Any general health concerns should be scheduled with your primary care provider. 

You are welcome to use your HSA/FSA. If you should require a superbill  there is surcharge for creating this document. Please note that while we are happy to provide a superbill for your records, submission of a superbill does not guarantee reimbursement or approval of the charge through your HSA/FSA account. Coverage and eligibility for reimbursement are determined by your individual plan provider, so we recommend confirming directly with them for specific requirements.
